Procedure for changing the implant

Preparation

During a detailed consultation, we will determine the best treatment option for you and inform you in detail about the procedure and possible risks. We advise you to refrain from taking blood-thinning medication and nicotine for at least 2 weeks before the operation to ensure optimal wound healing.

The intervention

The procedure is usually performed under general anesthesia and involves a hospital stay of 2 to 3 nights. Access for replacing the implant is via the existing scar. In the case of capsular fibrosis, the hardened tissue is removed.

Aftercare

You should rest your body for one to two weeks after the inpatient stay. You will be given a special support bra which you should wear for the next 6 weeks. Avoid sport and heavy lifting during this time.

Reasons for a breast implant change

Changing breast implants can make sense for both aesthetic and medical reasons. Health aspects and individual wishes play a decisive role here:

Implant change due to capsular fibrosis

After insertion of the breast implant, the body normally forms a capsule of connective tissue around the implant to hold it in place. In some women, this capsule formation leads to excessive hardening of the tissue, which is known as capsular fibrosis.

Capsular fibrosis in breast implants can be promoted by various factors. There is an increased risk if a hematoma or infection occurs after the operation. The placement of the implant also plays a role. If the implant is placed above the muscle, the risk of capsular fibrosis is higher than if it is placed below the muscle.

The coating of the implant is also important. Implants with a rough surface have a lower risk of capsular fibrosis than smooth-walled implants.

Classification according to Baker

Capsular fibrosis is classified according to Baker.

  • Baker I: A soft capsule surrounds the implant, no changes are visible.
  • Baker II: Slight hardening of the connective tissue, possible feelings of tension.
  • Baker III: Moderate hardening of the connective tissue, incipient deformation of the implant.
  • Baker IV: Shrinkage of the connective tissue around the implant, severe pain and considerable deformation of the breast.

If the implant lies under the muscle, the risk of capsular fibrosis after 10 years is around 10 %. Treatment with implant removal should be considered for Baker III-IV at the latest.

Further reasons for an implant change

Other possible reasons for a breast implant change include

  • Implant rupture or leakage: Modern implants are very resistant, but damage or rupture of the shell can occur over time. In this case, the implant should be removed and replaced immediately.
  • Desire for a new size or shape: Many women wish to have their implants adjusted over time, e.g. to achieve a more discreet or fuller décolleté.
  • Breast changes due to pregnancy or weight loss: Pregnancy or significant weight changes can affect the appearance of the breast. Changing the implants can help to optimize the breast shape.
  • Outdated implants: Older implants should be examined regularly. Even if there is no discomfort, it may make sense to change them in order to insert more modern, safer implants with a more natural feel.
  • Dissatisfaction with previous surgery: In some cases, patients are not satisfied with the results of their previous breast augmentation. Changing implants can help to achieve a more harmonious and aesthetic result.

Options for breast implant replacement

Depending on the initial situation and individual wishes, there are various options for changing breast implants.

Simple implant change

In a simple implant exchange, we remove the old implant and replace it with a new one. Other implant shapes or sizes can also be selected.

Implant replacement with breast lift

If the tissue has sagged over the years or the skin has lost its elasticity, a breast lift can also be performed. This lifts the breast and gives it a youthful, firmer shape.

Change to smaller implants or implant removal

Some patients decide to switch to smaller implants or to go without. In this case, a breast lift or breast augmentation with autologous fat (lipofilling) can help to achieve a harmonious and natural result.

Change after capsular fibrosis

In the case of capsular fibrosis (Baker grade III or IV), the implant and capsule must be surgically removed. A new implant can then be inserted or, alternatively, autologous fat treatment can be performed.

FAQ – Frequently asked questions about implant replacement in Zurich

A change may be necessary for medical or aesthetic reasons. Sometimes aesthetic expectations change and a change is desired. Medical reasons include, for example, capsular fibrosis and implant damage.

Capsular fibrosis is the most common complication of breast augmentation with implants. A hardened tissue capsule forms around the implant which, depending on the stage, can lead to pain and deformation of the breast.
