Preparation

In a detailed consultation, the initial situation, the available options and their procedure are discussed in detail. If desired, a 3D analysis can be carried out. We will clarify your wishes and expectations as well as possible risks and alternatives. If you smoke, you should stop smoking completely 3 weeks before and after the operation. You should also stop taking blood-thinning medication or dietary supplements before the operation. Arnica globules 7 days before and after the operation can promote the healing process.

The intervention

Bichectomy is performed on an outpatient basis – under local anesthesia, twilight sleep or general anesthesia, depending on the initial situation. The buccal fat is carefully removed via an approx. 1 cm long incision on the inside of the cheek. The wound is closed with absorbable sutures. The procedure is low-risk, involves little bleeding and usually takes less than 45 minutes.

Aftercare

In the first few days, soft or liquid food is recommended so as not to impair wound healing. We recommend antiseptic mouthwashes to prevent infection. Slight swelling or feelings of pressure are normal and usually subside within a week. After about 5-7 days you will be able to socialize again, sports are possible after 2 weeks.

Reasons for a
bichectomy

A fuller face with accentuated cheeks can occur regardless of body weight – often due to genetic predisposition. Fat deposits accumulate in the lower face in particular, making the face appear round or childlike. If you want more definition and a slimmer profile, a bichectomy – also known as buccal fat removal – can be used to remove stubborn fat from the cheeks. The result: visibly narrower cheeks, a clearer jawline and an overall more mature, distinctive appearance.

Methods for slimming the face

There are various invasive and minimally invasive methods for facial slimming. In a personal consultation, we will analyze which procedure is best suited to your needs.

Surgical removal of the cheek fat

As already mentioned at the beginning, bichectomy or buccal fat removal is a good way to permanently remove stubborn cheek fat. The procedure is performed via a small incision on the inside of the cheek, so the scar is invisible in the oral cavity; the suture is made with self-dissolving thread.

Botulinum treatment for facial slimming

Targeted injections into the masseter muscle (masseter) or the parotid gland can reduce the width of the face – especially in the case of a muscular jaw. The treatment is repeated every 6 weeks. After 2-3 sessions, the face becomes visibly narrower. The procedure is also suitable for treating bruxism (teeth grinding).

Liposuction, lipolysis and implants

Depending on the anatomy, liposuction in the lower jaw area or targeted lipolysis, i.e. fat injection, can also be performed. In some cases, a combination with a chin or cheek implant may also be advisable.

Bichectomy is a low-risk procedure when performed by a specialist, but possible complications such as infections, swelling, hematomas or temporary numbness in the mouth should be addressed in advance. Bichectomy is suitable for healthy adults with fuller cheeks and a stable body weight. Smokers, people with severe weight loss or a very thin face are not suitable candidates.
