    Cellulite affects a great many women—regardless of age, weight, or fitness level. It typically manifests as dimples, ripples, and unevenness in the skin, especially on the thighs, buttocks, hips, or upper arms.     The good news: There are now several scientifically proven treatment approaches that can be combined depending on the severity and individual circumstances. It’s important to note that not all cellulite is the same. For some women, mild, widespread dimpling is the main concern, while others experience deeper dimples, excess fat tissue, or a loss of skin firmness.

    That is precisely why there is no single “best” treatment for everyone. The key factor is always which structures are responsible for the visible changes in each individual case. In this article, we present six established methods for treating cellulite that we offer at our practice.

    Not all cellulite is the same

    Cellulite is caused by the interaction of connective tissue, fatty tissue, and skin. In women, the connective tissue strands run vertically. When fat cells enlarge between these structures, the tissue bulges outward, while the fibers pull the skin inward at certain points. This is how the typical dimples form.

    The extent to which cellulite is visible depends, among other things, on:

    • the quality of the connective tissue
    • skin thickness and skin elasticity
    • fat distribution
    • hormonal influences
    • genetic predisposition
    • lifestyle

    That is why not all forms of cellulite respond equally well to the same treatment.

    What improvements can a cellulite treatment realistically achieve?

    A modern cellulite treatment can:

    • visibly refine the skin’s texture,
    • Reduce dents and
    • improve the quality of the fabric.

    The goal is not unrealistic perfection, but a noticeable and visible improvement in the appearance of the skin. Which method is appropriate depends primarily on whether your condition is characterized primarily by mild to moderate, widespread cellulite, deep, fixed dimples, excess fat deposits, or a loss of skin firmness.

    What methods are there for treating cellulite?

    At Tremp Plastic Surgery, various procedures are available that can be used individually or in combination, depending on the patient’s specific condition.

    LPG® Endermology

    LPG® Endermologie is a non-invasive method that mechanically stimulates the tissue. Targeted rolling and suction movements stimulate blood circulation and lymphatic flow, which can visibly refine the skin’s texture.

    • Particularly suitable for:
      mild to moderate cellulite, as an adjunct treatment or as maintenance therapy
    • What you can realistically expect:
      an improvement in skin texture, smoother-feeling skin, and a slight tightening effect
    • Important to know:
      LPG® Endermologie is a gentle and supportive method. For deep, deeply embedded dimples, it is usually not sufficient on its own.

    Sculptra® Body

    Sculptra® Body is a collagen stimulator. It stimulates the body’s own collagen production over several weeks and months. This can improve skin quality, which reduces the appearance of cellulite dimples.

    • Particularly suitable for:
      patients with thinner skin, reduced skin elasticity, or a need for additional skin tightening
    • What you can realistically expect:
      firmer, more toned skin and a gradual improvement in your skin’s appearance
    • Important to know:
      Sculptra® Body primarily improves skin quality. It does not directly release deep connective tissue strands that pull the skin inward, causing dimples.

    Subcision

    Subcision directly targets the fixed strands of connective tissue beneath the skin. These strands are often responsible for pulling individual dimples significantly inward. By selectively releasing them, the skin in these areas can smooth out again.

    • Particularly suitable for:
      deep, pronounced, and set-in dents
    • What you can realistically expect:
      an often significant improvement in individual, clearly defined cellulite dimples
    • Important to know:
      Subcision is particularly effective when the dimples are structurally caused by hardened bands of tissue. This method is often combined with collagen-stimulating treatments.

    VASER® liposuction

    VASER® liposuction uses ultrasound energy to gently break up and remove fat tissue. This procedure is particularly suitable when cellulite is combined with localized fat deposits and the patient also wishes to improve body contour.

    • Particularly suitable for:
      Cellulite accompanied by excess fat and a desire for a more defined body contour
    • What you can realistically expect:
      a reduction in fat volume and a smoother, more harmonious silhouette
    • Important to know:
      VASER® primarily targets fat volume and contour. It does not directly break up deep connective tissue strands, which is why combining it with other methods is often advisable.

    Argon Plasma Coagulation

    Argon plasma coagulation may be beneficial for cellulite accompanied by sagging skin. Heating the tissue helps to tighten the skin, while also stimulating collagen production.

    • Particularly suitable for:
      Cellulite combined with sagging skin
    • What you can realistically expect:
      improved tissue firmness and a noticeable smoothing of the skin’s appearance
    • Important to know:
      This method is often used as a complementary treatment, for example, in combination with liposuction.

    Radiofrequency microneedling

    Radiofrequency microneedling specifically stimulates the skin to promote collagen and elastin production. This can improve the skin’s structure and make its appearance finer and smoother.

    • Particularly suitable for:
      mild to moderate cellulite, with a focus on skin quality and surface texture
    • What you can realistically expect:
      a visible improvement in skin texture and some firming
    • Important to know:
      For deep, persistent acne scars, radiofrequency microneedling is not a substitute for subcision, but it can be a very useful complementary treatment.

    Treating Cellulite: Which Treatment Is Right for Which Condition?

    To put it simply:

    • Mild to moderate cellulite: often treated with LPG® Endermologie or radiofrequency microneedling
    • Thinner skin and reduced elasticity: often Sculptra® Body
    • Deep, fixed indentations: usually subcision
    • Cellulite plus localized fat deposits: often treated with VASER® liposuction
    • Cellulite and sagging skin: supplemented with argon plasma coagulation
    • More complex findings: often a combination of several procedures

    Why Combinations Often Yield the Best Results

    Especially in cases of more severe cellulite, a combination of treatments is often the most effective solution. This is because it’s not uncommon for several factors to interact at the same time.

    In practice, it is often the case that a single method does not address all the causes of cellulite at the same time. For example, a deep dimple may be caused by strands of connective tissue, while at the same time the skin has become thinner and less elastic. Or there may also be localized fat deposits that accentuate the unevenness even more.

    That is exactly why a combined treatment approach can be beneficial, for example:

    • Subcision to release deep strands
    • Sculptra® Body, to improve skin quality
    • LPG® Endermologie or radiofrequency microneedling to further refine the skin’s texture
    • VASER® Liposuction for Excess Fat
    • Argon Plasma Coagulation for Concurrent Skin Tightening

    What can I do on my own to combat cellulite?

    Exercise, maintaining a stable body weight, staying well-hydrated, and eating a balanced diet can have a positive effect on the appearance of the skin and support treatment results. Nevertheless, it’s important to know that cellulite is primarily caused by structural factors. For this reason, pronounced dimples often cannot be permanently eliminated through exercise, weight loss, or creams alone.

    Cellulite can be significantly reduced, and skin texture can be visibly improved. However, due to the underlying anatomy, complete and permanent elimination cannot realistically be guaranteed—the focus is on a noticeable and visible improvement.

    The procedure is performed under local anesthesia, so it is generally well tolerated. After the procedure, tenderness, swelling, and bruising are normal for a few days to a few weeks.

    To achieve visible results, a series of 10–15 sessions is usually recommended, followed by monthly maintenance treatments.

    The active ingredient is poly-L-lactic acid (PLLA). It is often combined with subcision, which is a sensible approach: Subcision addresses the mechanical cause of the dimple, and Sculptra Body then improves the skin’s quality and thickness over the treated area.

    There is no set age limit. The key factor is the individual assessment. During a personal consultation, the severity of cellulite is evaluated, and the appropriate method or combination of methods is recommended.

    The plasma energy heats the connective tissue, causing an immediate contraction of the fibers and leading to increased collagen and elastin production over the course of several months. It is most often used as a complementary treatment to liposuction. The exact mechanism of action has not yet been conclusively established scientifically, and the body of research is even more limited compared to subcision or Sculptra® Body—the method is therefore considered more of a complementary procedure than a standalone one.

    No, the two methods work differently. RF microneedling improves the skin’s texture and quality across a broad area by stimulating collagen and elastin production, while subcision specifically cuts through the fixed strands of connective tissue that cause deep dimples. For severe, fixed cellulite, subcision is usually more effective; RF microneedling is well-suited as a complementary treatment or for milder cases of cellulite.
