Buttock Augmentation and Buttock Lift: Methods, Results, and What You Should Know

Gesäss einer Frau, symbolisiert Gesässvergrösserung

The buttocks are one of the most defining features of the body’s silhouette—and also one of the areas most visibly affected by weight fluctuations, genetic predisposition, and the natural aging process. Loss of volume, sagging skin, and lack of projection can have different causes—and therefore require different solutions.

Modern aesthetic surgery now offers a range of advanced techniques that can be precisely tailored to the individual anatomy. In the following sections, you will learn what each method can achieve, how they differ, and what to consider when choosing the right approach.

Buttock Augmentation and Buttock Lift: An Overview of the Four Methods

Modern aesthetic surgery offers a range of refined approaches to enhance the contour of the buttocks. The choice of the optimal method for buttock augmentation depends on your individual anatomy, body fat percentage, desired outcome, and lifestyle. During a personal consultation, we will work together to determine which approach best aligns with your goals.

    Fat Transfer (BBL)

    • Your own fat is harvested, processed, and precisely injected.
    • The most natural-looking result.

    Implants

    • Silicone implants provide defined volume—ideal for patients with little body fat.
    • Long-lasting and shape-stable.

    Hybrid Method

    • A combination of implants and fat transfer for an optimized overall result.
    • Maximum flexibility.

    Buttock Lift

    • Removal of excess skin, tightening of soft tissue, and contour refinement.
    • Especially suitable after significant weight loss.

    1. Fat Transfer Buttock Augmentation (Brazilian Butt Lift – BBL)

How does the BBL procedure work?

In a Brazilian Butt Lift (BBL), the patient’s own fat is first gently harvested through liposuction—typically from areas such as the abdomen, flanks, hips, or thighs. The extracted fat is then purified and processed using centrifugation to separate viable fat cells from excess fluid and impurities.

After preparation, the purified fat is carefully injected in multiple layers into the buttocks. This allows for precise shaping, improved volume, and a natural-looking contour. An additional benefit is the slimming effect in the donor areas, resulting in an overall more balanced body silhouette.

Advantages of the Fat Transfer Method

Since only the patient’s own tissue is used, there is no risk of rejection or allergic reaction. Scarring is minimal, and the result feels completely natural while moving harmoniously with the body. In addition, patients benefit from the body-contouring effect of liposuction in the donor areas.

Important note: A successful BBL requires a sufficient amount of available body fat. In very slim patients, the amount of transferable fat may be limited. Approximately 30–50% of the injected fat is naturally reabsorbed by the body over time—this is carefully taken into account during treatment planning.

Procedure and Recovery of the Fat Transfer Method

The procedure is performed under general anesthesia and typically takes around 2–4 hours, depending on the extent of treatment. In the first weeks after surgery, a special compression garment must be worn to support healing and reduce swelling.

Direct pressure on the buttocks should be avoided for approximately 4–6 weeks to ensure optimal fat survival and integration. The final result becomes fully visible after about 3–6 months, once the swelling has subsided and the transferred fat has stabilized.

    2. Buttock Implants

    Who are implants suitable for?

Buttock implants made of solid, cohesive silicone are particularly suitable for slim patients who do not have sufficient body fat for a Brazilian Butt Lift (BBL). They allow for precise and long-lasting volume enhancement and are available in various shapes and sizes, enabling a tailored approach to individual anatomical needs and aesthetic goals.

Surgical Technique: Buttock Augmentation with Implants

The incision is made discreetly in the gluteal crease (intergluteal fold), allowing for a nearly invisible scar once healed. The implant is typically placed in a submuscular or intramuscular pocket, which ensures a natural appearance while also providing optimal protection of the implant.

Long-term results: Modern buttock implants are durable and designed to withstand everyday mechanical stress such as sitting and physical activity. Nevertheless, regular follow-up examinations are recommended. In rare cases, implant replacement or removal may become necessary.

    3. Hybrid Method: Implant + Fat Transfer

The hybrid method combines the advantages of both procedures: an implant provides the structural base volume, while autologous fat is used to refine contours, smooth transitions, and correct minor irregularities. This technique allows for highly individualized and harmonious body contouring and is particularly suitable for patients who desire significant volume enhancement while still aiming for the most natural-looking result possible.

    4. Buttock Lift (Lower Body Lift / Gluteal Lift)

A buttock lift focuses on removing excess skin and tightening the soft tissues in the buttock and lower back region. This procedure improves the overall body contour of the lower trunk and, depending on the individual findings, may also include the outer and inner thighs.

Unlike volume-enhancing procedures such as the Brazilian Butt Lift, the primary goal of a buttock lift is not augmentation but tightening and reshaping the tissue to restore a firmer, more youthful silhouette.

When is a buttock lift advisable?

A buttock lift is recommended when excess, loose skin predominantly affects the appearance—typically after significant weight loss (e.g. following bariatric surgery) or as part of the natural aging process. The procedure tightens the skin and elevates the position of the buttocks without necessarily adding volume.

Combination of buttock lift with volume-enhancing techniques

In many cases, a buttock lift is combined with fat transfer or implants in order to both tighten the skin and restore or enhance volume. This combined approach allows for particularly balanced and harmonious body contouring results.

FAQ – Frequently Asked Questions about Butt Augmentation

Implant-based results are permanent but may require replacement over time. In a BBL, the fat that successfully integrates (approx. 50–70%) remains permanent, although weight fluctuations can influence the outcome. A buttock lift also provides long-lasting results, but is still subject to the natural ageing process of the skin.

In general, the outcome of a butt augmentation is stable over the long term. Maintaining a stable body weight and a healthy lifestyle significantly helps preserve the achieved shape. Natural ageing processes may gradually affect the tissue, but the aesthetic improvement typically remains visible for many years.

The procedure is performed under general anaesthesia. In the first few days after surgery, a feeling of pressure and tightness is normal and can be well managed with pain medication. Most patients experience the recovery phase as more comfortable than expected.

To ensure optimal fat survival, direct pressure on the buttocks should be avoided for approximately 4–6 weeks. Special BBL cushions allow sitting without putting pressure on the treated area. When sleeping, a prone (on the stomach) or side position is recommended.

Light activity is possible after about two weeks and is even beneficial for recovery. However, intensive exercise – especially squats, running, and weight training – should only be resumed after 6–10 weeks, depending on the method used and individual healing progress.

The surgical access is made through a small incision in the natural gluteal fold (intergluteal crease), which is completely concealed by underwear in everyday life and usually heals very discreetly.

The achievable volume depends on your aesthetic goals, the chosen method, and your individual anatomy. While implants provide a precisely defined volume, the result of a BBL is limited by the amount of available body fat.

Aesthetic procedures for buttock enhancement or reshaping are generally not covered by statutory health insurance and must be paid for privately. Exceptions may apply in cases of medically indicated reconstructive surgery. We will discuss all costs transparently during your consultation.

In the four weeks before surgery, you should avoid nicotine and blood-thinning medications. A stable body weight and good general health support optimal results. Detailed pre-operative instructions will be provided individually during your consultation.

Non-surgical options such as targeted muscle training can improve overall shape but cannot replicate surgical results. Some filler-based approaches exist, but they are not routinely recommended due to safety considerations. For long-lasting aesthetic results, surgery remains the gold standard.

Suitability depends on several factors, including your general health, body fat percentage, and aesthetic goals. While a BBL requires sufficient body fat, implants or hybrid techniques may also be suitable for slimmer patients. A personalised assessment is carried out during an individual consultation, where all options are discussed transparently in a trusted environment.
